This pilot randomized controlled trial explored the feasibility, acceptability, and potential utility of an intervention to improve the decisional capacity (DC) for research consent/assent among youth living with HIV (YLWH). We randomly assigned 30 YLWH aged 13–24 to a control (paper-based) or intervention (multimedia) consent/assent condition. Using a between- and within-subjects design, participants completed a demographic questionnaire and measures of DC, acceptability/feasibility, and voluntary self-consent perceptions after undergoing a hypothetical consent/assent process for a biomedical HIV study. Findings suggest participants found (...) the intervention feasible and acceptable. Participants in the intervention condition scored higher on a DC measure; however, scores did not meet the predetermined cutoff score for adequate DC in either group. This paper shows that, for a large range of parameters, the journal editor prefers to delegate the choice to review the manuscript to the biased referee. If the peer review process is informative and the review reports are costly for the reviewers, even biased referees with extreme scientific preferences may choose to become informed about the manuscript’s quality. On the contrary, if the review process is potentially informative but the reviewer reports are not costly for the referees, the biased reviewer (...) has no incentive to become informed about the manuscript. Furthermore, if the reports are costly for referees but the peer review processes are not potentially informative, the biased reviewers will never become informed. Seguidamente são analisadas a recepção e a transformação do pensamento kantiano a partir de duas (...) correntes filosóflcas actuais: o Racionalismo Crítico de Karl Popper e Hans Albert, e a Pragmática Transcendental da linguagem de Karl-Otto Apel. Em ambos os casos se compara o sen modo próprio de incorporar as noções de crítica e transcendentalidade com a concepção original de Kant. /// The article addresses the influence of Kantian philosophy in contemporary thought from the view point of the crisis of illustrated Modernity. The authors start by analyzing the original Kantian position, whereby particular attention is given to his conception of criticism and transcendentalism. This original article outlines a theoretical path and posterior critical analysis regarding two relevant matters in modern nursing: patterns of knowing in nursing and commodification contexts in contemporary health systems. The aim of our manuscript is to examine the development of basic and contextual nursing knowledge in commodified contexts. For this purpose, we outline a discussion and reflexive dialogue based on a literature search and our clinical experience. To lay the foundation for an informed discussion, we conducted a literature search (...) and selected relevant articles in English, Spanish, and Portuguese that included contents on patterns of knowing, commodification, and nursing published from 1978 to 2017. Globalization, commodification, and austerity measures seem to have negative effects on nursing. Work conditions are worsening, deteriorating nurse–patient relationships, and limiting reflection on practice. Nurses must develop knowledge to challenge and participate in institutional organization and public health policies. Development of nursing knowledge may be difficult to achieve in commodified environments. Consequently, therapeutic care relationships, healthcare services, and nurses' own health are compromised. However, by obtaining organizational, sociopolitical, and emancipatory knowledge, nurses can use strategies to adapt to or resist commodified contexts while constructing basic knowledge. (shrink)

The CG and labour management of firms are closely related. A particularly productive way to study how CG affects and is affected by the employment relationship has been to compare CG across countries. The contributions of this paper to that literature are threefold. (1) An integration of aspects of the labour (...) management literature in the CG debate. (2) Based on a sample of about 1000 firms from 31 countries, we find evidence of complementarities between the CG and the labour management of firms. Extreme cases, in general, outperform mixed cases. (3) Firm differences within countries are more important than scholars have assumed so far. The commodification of health care, particularly primary care, presents challenges to care and knowledge development. The purpose of this study is to examine how nurses perceive and develop their knowledge in a commodified context. A mixed‐methods study was conducted that included a closed‐question survey and in‐depth interviews with nurses in public primary care in Catalonia. There were 104 valid responses to the questionnaire and 10 in‐depth interviews. The main findings of the survey were related to workload and limited time for (...) nursing care. Six themes emerged from the in‐depth interviews: (1) limited time for nursing, (2) feelings of burnout, (3) awareness of patient and family satisfaction, (4) organizational factors that favor nurses' needs, (5) organizational factors that hinder nurses' needs, and finally (6) public administration requirements. Participants perceive excessive workload and time constraints and feel that this affects their nursing care and their physical and mental health. However, nurses purposefully use knowledge patterns to cope with the problems associated with commodification. Nurses have multidimensional, contextualized, and integrated knowledge that allows them to optimize their care based on the needs of their patients. This research examines many challenges related to nursing practice and the nursing discipline and opens the door for further research that encompasses all areas of nursing. (shrink)

This article will explore a clinical case study of a home visit carried out by the case manager nurse. In this case, we will discuss the dilemma of finding the balance between autonomy and beneficence from the perspective of principlist ethics, virtue ethics and the ‘ethics of care’. The main conflict in this case study deals with all proposals are unsuitable and it is not necessary for a nurse to pay him a home visit, whereas for the healthcare system it (...) is considered necessary. We could conclude that, during the home visit, the case manager aspires to achieve excellence, and throughout his clinical relationship with Francesc, searches for a series of virtues, respecting certain fundamental principles. In this way, the case managers ensure that Jaume’s care is more humanised. The case has been anonymised and confidentiality maintained. (shrink)

DNA methylation constitutes one of the pillars of epigenetics, relying on covalent bonds for addition and/or removal of chemically distinct marks within the major groove of the double helix. DNA methyltransferases, enzymes which introduce methyl marks, initially evolved in prokaryotes as components of restriction‐modification systems protecting host genomes from bacteriophages and other invading foreign DNA. In early eukaryotic evolution, DNA methyltransferases were horizontally transferred from bacteria into eukaryotes several times and independently co‐opted into epigenetic regulatory systems, primarily via establishing connections (...) with the chromatin environment. While C5‐methylcytosine is the cornerstone of plant and animal epigenetics and has been investigated in much detail, the epigenetic role of other methylated bases is less clear. The recent addition of N4‐methylcytosine of bacterial origin as a metazoan DNA modification highlights the prerequisites for foreign gene co‐option into the host regulatory networks, and challenges the existing paradigms concerning the origin and evolution of eukaryotic regulatory systems. Unfortunately, there are still cases of domestic violence or situations where it is necessary to call for help without arousing the suspicion of the aggressor. In these situations, the help signal devised by the Canadian Women’s Foundation has proven to be effective in reporting a risky situation. By displaying a sequence of hand signals, it is possible to report that help is needed. This work presents a vision-based system that detects this sequence and implements it in a social robot, so (...) that it can automatically identify unwanted situations and alert the authorities. The gesture recognition pipeline presented in this work is integrated into a cognitive architecture used to generate behaviours in robots. In this way, the robot interacts with humans and is able to detect if a person is calling for help. In that case, the robot will act accordingly without alerting the aggressor. The proposed vision system uses the MediaPipe library to detect people in an image and locate the hands, from which it extracts a set of hand landmarks that identify which gesture is being made. By analysing the sequence of detected gestures, it can identify whether a person is performing the distress hand signal with an accuracy of 96.43%. (shrink)

A total of 1918 loci, detected by the hybridization of 938 expressed sequence tag unigenes from 26 Triticeae cDNA libraries, were mapped to wheat homoeologous group 4 chromosomes using a set of deletion, ditelosomic, and nulli-tetrasomic lines. The 1918 EST loci were not distributed uniformly among the three group 4 chromosomes; 41, 28, and 31% mapped to chromosomes 4A, 4B, and 4D, respectively. This pattern is in contrast to the cumulative results of EST mapping in all homoeologous groups, as reported (...) elsewhere, that found the highest proportion of loci mapped to the B genome. Sixty-five percent of these 1918 loci mapped to the long arms of homoeologous group 4 chromosomes, while 35% mapped to the short arms. The distal regions of chromosome arms showed higher numbers of loci than the proximal regions, with the exception of 4DL. This study confirmed the complex structure of chromosome 4A that contains two reciprocal translocations and two inversions, previously identified. An additional inversion in the centromeric region of 4A was revealed. A consensus map for homoeologous group 4 was developed from 119 ESTs unique to group 4. Forty-nine percent of these ESTs were found to be homoologous to sequences on rice chromosome 3, 12% had matches with sequences on other rice chromosomes, and 39% had no matches with rice sequences at all. Limited homology was found between wheat ESTs on homoeologous group 4 and the Arabidopsis genome. Forty-two percent of the homoeologous group 4 ESTs could be classified into functional categories on the basis of blastX searches against all protein databases. (shrink)

Imagination has received a great deal of attention in different fields such as psychology, philosophy and the cognitive sciences, in which some works provide a detailed account of the mechanisms involved in the creation and elaboration of imaginary worlds. Although imagination has also been formalized using different logical systems, none of them captures those dynamic mechanisms. In this work, we take inspiration from the Common Frame for Imagination Acts, that identifies the different processes involved in the creation of imaginary worlds, (...) and we use it to define a dynamic formal system called the Logic of Imagination Acts. We build our logic by using a possible-worlds semantics, together with a new set of static and dynamic modal operators. The role of the new dynamic operators is to call different algorithms that encode how the formal model is expanded in order to capture the different mechanisms involved in the creation and development of imaginary worlds. We provide the definitions of the language, the semantics and the algorithms, together with an example that shows how the model is expanded. By the end, we discuss some interesting features of our system, and we point out to possible lines of future work. (shrink)

The activism of institutional investors tends more and more toward the supervision and control of the behavior of the managers of big companies. In this article, we present a model based on the creation of an activism index that lets us evaluate such activism's effect on the sensitivity of the investment policies of a company in the face of financial variables and market variables. To test our assertions, we analyze firm-level data for United Kingdom, Germany, France, Denmark, and Spain during (...) the period 1995-2004. Our results point to a significant reduction in the sensitivity of company investment decisions in the face of these variables, especially relative to intangible capital, as a result of the neutralizing effect of activism on the high agency costs of free cash flow and on the information asymmetries of the market. The aim of our research is to analyze how different religions influence business ethics. We develop an index of practices in the field of business ethics, made up of 19 items containing practices related to workers, consumers, products, human rights, management of ethical conflicts, and crime prevention. Also, we consider a wide range of religion affiliations. To undertake this research, we use a panel data sample composed of 11,956 firm‐year observations from 18 countries. Drawing on stakeholder theory, we posit some (...) hypotheses based on the religions considered. The results obtained show that a higher percentage of religious adherents in the country where a company does business usually involve the implementation of more ethical practices in the corporate field. These findings are obtained for Christian, Islamic, Jewish, and Hindu religions. In contrast, this does not stand for Buddhist and folk religions. Also, countries where there is no predominant religion do not show a confluence between religion and ethical corporate practices. (shrink)

This article explores whether and how the biotechnologization process that the fuel-plant Jatropha curcas is undergoing might strengthen local sustainable development. It focuses on the ongoing efforts of the multi-stakeholder network Gota Verde to harness Jatropha within local small-scale production systems in Yoro, Honduras. It also looks at the genomics research on Jatropha conducted by the Dutch research institute Plant Research International, specifically addressing the ways in which that research can assists local development in Honduras. A territorial approach is applied (...) for analysis employing a three domain concept (local sustainable biotechnological development) of territory, technology and re-territorialization. The article suggests that, although the biotechnologization process (through genomics) of Jatropha within the socio-technical framework of the institute and multi-stakeholder networks is an ongoing process––and different trajectories are, therefore, still open––the process can, nevertheless, strengthen local sustainable development. Critical thinking is a complex, dynamic process formed by attitudes and strategic skills, with the aim of achieving a specific goal or objective. The attitudes, including the critical thinking attitudes, constitute an important part of the idea of good care, of the good professional. It could be said that they become a virtue of the nursing profession. In this context, the ethics of virtue is a theoretical framework that becomes essential for analyse the critical thinking concept in nursing care and (...) nursing science. Because the ethics of virtue consider how cultivating virtues are necessary to understand and justify the decisions and guide the actions. Based on selective analysis of the descriptive and empirical literature that addresses conceptual review of critical thinking, we conducted an analysis of this topic in the settings of clinical practice, training and research from the virtue ethical framework. Following JBI critical appraisal checklist for text and opinion papers, we argue the need for critical thinking as an essential element for true excellence in care and that it should be encouraged among professionals. The importance of developing critical thinking skills in education is well substantiated; however, greater efforts are required to implement educational strategies directed at developing critical thinking in students and professionals undergoing training, along with measures that demonstrate their success. Lastly, we show that critical thinking constitutes a fundamental component in the research process, and can improve research competencies in nursing. We conclude that future research and actions must go further in the search for new evidence and open new horizons, to ensure a positive effect on clinical practice, patient health, student education and the growth of nursing science. We define a multi-modal version of Computation Tree Logic ( ctl ) by extending the language with path quantifiers E δ and A δ where δ denotes one of finitely many dimensions, interpreted over Kripke structures with one total relation for each dimension. As expected, the logic is axiomatised by taking a copy of a ctl axiomatisation for each dimension. Completeness is proved by employing the completeness result for ctl to obtain a model along each dimension in turn. We also (...) show that the logic is decidable and that its satisfiability problem is no harder than the corresponding problem for ctl. We then demonstrate how Normative Systems can be conceived as a natural interpretation of such a multi-dimensional ctl logic. (shrink)

One of the objectives in the field of artificial intelligence for some decades has been the development of artificial agents capable of coexisting in harmony with people and other systems. The computing research community has made efforts to design artificial agents capable of doing tasks the way people do, tasks requiring cognitive mechanisms such as planning, decision-making, and learning. The application domains of such software agents are evident nowadays. Humans are experiencing the inclusion of artificial agents in their environment as (...) unmanned vehicles, intelligent houses, and humanoid robots capable of caring for people. In this context, research in the field of machine ethics has become more than a hot topic. Machine ethics focuses on developing ethical mechanisms for artificial agents to be capable of engaging in moral behavior. However, there are still crucial challenges in the development of truly Artificial Moral Agents. This paper aims to show the current status of Artificial Moral Agents by analyzing models proposed over the past two decades. As a result of this review, a taxonomy to classify Artificial Moral Agents according to the strategies and criteria used to deal with ethical problems is proposed. The presented review aims to illustrate (1) the complexity of designing and developing ethical mechanisms for this type of agent, and (2) that there is a long way to go (from a technological perspective) before this type of artificial agent can replace human judgment in difficult, surprising or ambiguous moral situations. Physician behavior in real-life pediatric consultations in relation to ethical practice, such as informed consent (provision of information, understanding), respect for integrity and patient autonomy (decision-making), has not been subjected to thorough empirical investigation. Such investigations are important tools in developing sound ethical praxis.

Word co‐occurrence patterns in language corpora contain a surprising amount of conceptual knowledge. Large language models (LLMs), trained to predict words in context, leverage these patterns to achieve impressive performance on diverse semantic tasks requiring world knowledge. An important but understudied question about LLMs’ semantic abilities is whether they acquire generalized knowledge of common events. Here, we test whether five pretrained LLMs (from 2018's BERT to 2023's MPT) assign a higher likelihood to plausible descriptions of agent−patient interactions than to minimally (...) different implausible versions of the same event. Using three curated sets of minimal sentence pairs (total n = 1215), we found that pretrained LLMs possess substantial event knowledge, outperforming other distributional language models. In particular, they almost always assign a higher likelihood to possible versus impossible events (The teacher bought the laptop vs. The laptop bought the teacher). However, LLMs show less consistent preferences for likely versus unlikely events (The nanny tutored the boy vs. The boy tutored the nanny). In follow‐up analyses, we show that (i) LLM scores are driven by both plausibility and surface‐level sentence features, (ii) LLM scores generalize well across syntactic variants (active vs. passive constructions) but less well across semantic variants (synonymous sentences), (iii) some LLM errors mirror human judgment ambiguity, and (iv) sentence plausibility serves as an organizing dimension in internal LLM representations. Overall, our results show that important aspects of event knowledge naturally emerge from distributional linguistic patterns, but also highlight a gap between representations of possible/impossible and likely/unlikely events. En este texto se aborda el vínculo entre la estructura narrativa de la memoria colectiva (Halbwachs, 2004b) y la dimensión hermenéutica de la ciudadanía (Valderrama, 2010), desde los hallazgos de la investigación denominada Configuraciones de memoria colectiva en participantes del Programa Prensa Escuela entre 2015 y 2019. Para estos efectos, se aplicó una metodología cualitativa, de diseño de revisión documental, y se analizaron los textos de jóvenes, recopilados en la publicación anual El Taller. Se identificó que los temas desarrollados en (...) las narraciones en los cuales se vincula la memoria colectiva y la ciudadanía son identidad y territorio, marcos comunes de pensamiento y reconstrucción del pasado. De esta manera, la narración facilita el reconocimiento de la realidad y sus problemáticas, toda vez que los jóvenes asistentes al Programa podían expresar su percepción del mundo, confrontar elementos de su entorno y darle lugar al otro. Estos textos adquieren resonancia en cuanto entra la hermenéutica a tender puentes para la comprensión del mundo y en el caso de la ciudadanía para la construcción de nuevos modos de convivir. Children ‘born of war’ refer to people of any age conceived as the result of sexual violence at the hands of armed forces or groups during war, displacement, genocide or military occupation. Due to the circumstances of their birth, children ‘born of war’ can experience social stigma, discrimination and exclusion, resulting in diminished life chances and opportunities. At the same time, children ‘born of war’ fall through the cracks of global policy frameworks. In this article, we explore the reasons for (...) this, arguing that the nature of the harm these children endure renders their status as a victim group elusive. We propose a survivor-centred approach drawing on the lived experiences of children ‘born of war’. The approach recognizes the agency of children and draws attention to their expressed desire to contribute to, and participate in, processes of social reconstruction and reconciliation. (shrink)

In 1932, John von Neumann argued for the equivalence of the thermodynamic entropy and −Trρlnρ, since known as the von Neumann entropy. Meir Hemmo and Orly R. Shenker recently challenged this argument by pointing out an alleged discrepancy between the two entropies in the single-particle case, concluding that they must be distinct. In this article, their argument is shown to be problematic as it allows for a violation of the second law of thermodynamics and is based on an incorrect calculation (...) of the von Neumann entropy. To assess—from a qualitative perspective—the perceptions and attitudes of Spanish rehabilitation professionals (e.g. rehabilitation doctors, speech therapists, physical therapists) about Brain–Computer Interface (BCI) technology. A qualitative, exploratory and descriptive study was carried out by means of interviews and analysis of textual content with mixed generation of categories and segmentation into frequency of topics. We present the results of three in-depth interviews that were conducted with Spanish speaking individuals who had previously completed a survey as part of a larger, 3-country/language, survey (...) on BCI perceptions. 11 out of 15 of these Spanish respondents (survey) either strongly or somewhat accept the use of BCI in rehabilitation therapy. However, the results of our three in-depth interviews show how, due to a strong inertia of attitudes and perceptions about BCI technology, most professionals feel reluctant to use BCI technology in their daily practice (interview). Se realizó una revisión bibliográfica para profundizar en el estudio de la sexualidad en la adolescencia y las acciones educativas como alternativa de la educación sexual. Se ejecutó una búsqueda en la literatura digital de artículos de investigaciones originales en las bases de datos (...) SciELO Cuba, SciELO Regional, ScienceDirect, ClinicalKey, Cumed, Lilacs, así como los sitios web Organización Panamericana de la Salud, Organización Mundial de la Salud, Fondo de Población de las Naciones Unidas de Cuba, Fondo de las Naciones Unidas. Se concluye que las acciones educativas sobre sexualidad en la adolescencia permitirán a los jóvenes tomar decisiones fundamentadas sobre su sexualidad y su salud, prepararse para la vida y mantener una conducta sexual responsable. Meanwhile, adolescence, as a stage of humans' life, is characterized by biological, psychological and social changes. In order to deepen on the study of sexuality in adolescence and on educational actions as alternatives of sexual education, a bibliographic review was carried out. It was made a digital literature search of articles from original researches in SciELO Cuba, SciELO Regional, ScienceDirect, ClinicalKey, Cumed, Lilacs, as well as the Pan-American Health Organization, the World Health Organization, the United Nations Fund for Population Activities, and the United Nations Fund websites. It was reached the conclusion that educational actions about sexuality in adolescence will allow young people to make conscious decisions about their sexuality and health, prepare for life and maintain a responsible sexual behavior. La experiencia de hacer filosofía en la escuela como práctica de libertad nos convoca al análisis de los modos de recepción de la filosofía en la Argentina y Latinoamérica. Dicho de otra manera, el planteo de Filosofía con Niñ@s requiere reponer el problema de filosofía latinoamericana o filosofía en Latinoamérica; y, con ello, tematizar la "antinomia/oposición/ alternativa" de estos términos en relación con prácticas filosóficas no reproductivas, de gratuidad del pensar, de anulación de la asimetría a la hora del filosofar.

The objective of this article is to analyze the production of the subject Woman by reviewing some practices, discourses, and technologies promoted by the state, the church, and elites. It is important to emphasize that in most research about women or femininity, female subjectivity appears tightly linked to sexual difference. However, in this work I want to show that the notion of Woman is co-determined by race and class. The experience characteristic of such representation was possible only for a small (...) group of white and bourgeois women. Others—Indian, black, and mestiza “women”—could hardly account for a social experience comparable to the Western narrative about woman. Nevertheless, processes of homogenization allow these others to be classified and disciplined according to the gender norm, yet without altering the prejudices and inequalities produced by the prevailing racist and classist system, which implies the production of other female subjectivities, of other “women.”. This study focuses on taxonomic and typological methods of innovation policies in the European institutional context. Although many types of policies affect innovation, no universally accepted criteria exist to classify them. As innovation policy in a myriad of thematic areas—systemic model—has become pluralized, this article offers a method for classification. Such policies are grouped and categorized according to biological and neofunctional approaches.

The Convention on the Rights of Persons with Disabilities, approved on December 13, 2006 by the UN General Assembly, represented a historic advance in the field of disability by recognizing that the right to education implied the right to an quality inclusive education, thus consolidating a normative evolution from previous international instruments. Since May 3, 2008, the Convention has been part of the Spanish legal system, which implies that the State is obliged to modify national laws that do not correspond (...) to the principles established in the Convention. However, different reports have revealed the weakness of its implementation in Spain, with the consequent breach of the rights of people with disabilities. These difficulties in establishing a truly inclusive education show how ableist conceptions and practices continue to be perpetuated in the educational field. (shrink)

Con una indagación cualitativa se intenta averiguar si el método de palabras generatrices de Paulo Freire presenta ventajas para la enseñanza-aprendizaje de la lengua escrita castellana en la edad infantil. Se extraen los datos de la experiencia práctica acumulada por profesorado que lo ha aplicado en el segundo ciclo. Cinco maestras aportan informes escritos individuales y dos equipos docentes se constituyen en grupos de discusión. Tras la categorización y análisis de los datos, se concluye que el método tiene muchas ventajas. (...) Entre las ventajas de tipo didáctico cabe resaltar el respeto al protagonismo del alumnado y a su pensamiento sincrético, ser muy motivador, conseguir la comprensión lectora desde el principio, desarrollar simultáneamente las conciencias léxica, silábica y fonológica; enriquecer el vocabulario, fijar una ortografía correcta, integrar todos los ámbitos de conocimiento, y desarrollar la capacidad simbólica y la creatividad. Como ventajas de tipo pedagógico: considerar los efectos de tipo actitudinal que actúan sobre los procesos cognoscitivos, adaptarse a los contextos, permitir al profesorado conocer mejor a su alumnado, propiciar el desarrollo de la dimensión afectivo-emocional, favorecer la socialización por trabajarse siempre en grupo y la ayuda entre iguales, el buen clima de aprendizaje y la colaboración familiar. En el presente artículo, se examinan y discuten dos argumentos con consecuencias reduccionistas debidos a Jaegwon Kim y a Theodore Sider respectivamente. De acuerdo con el argumento de Kim, la superveniencia fuerte implicaría la coexistencia necesaria de propiedades (es decir, tal y como normalmente se interpreta, la reducción). De acuerdo con el de Sider, ocurriría lo mismo con la superveniencia global. Uno y otro hacen un uso esencial de sendas nociones de propiedad maximal, las cuales son discutidas aquí a la (...) luz de una interpretación natural e interesante de la teoría de las propiedades implícita en sus argumentos. Bajo esta nueva interpretación, en términos modelo-teóricos (véase apartado 4), obtenemos diversas posibilidades de relaciones formales entre las tesis de superveniencia y la reducción, según la lógica utilizada. Al menos bajo una interpretación interesante, los argumentos de Kim y Sider no son correctos, quedando demos-trado así que dichos argumentos no son válidos en general. We discuss and analyze two reductive arguments due to Jaegwon Kim and Theodore Sider respectively. According to the first one, strong supervenience would imply necessary coextension of properties (i.e., reduction). According to the second, this would be also the case of global supervenience. Kim and Sider make essential use of their respective notions of maximal properties, which we analyze here in the light of a natural and interesting interpretation of the underlying theory of properties. Under this interpretation, in terms of model theory (see § 4), we obtain different possibilities of formal relations between the superveniencie theses and reduction, depending on the logic we use. Under at least one interesting interpretation, the arguments of Kim and Sider are not correct and we become the conclusion that these arguments are not valid in general. (shrink)

RESUMEN Fundamentación: la intervención de los anestesiólogos durante el preoperatorio de los pacientes con fractura del tercio proximal del fémur no queda reducida a la recopilación de datos científicos de carácter biológico, es una exigencia actual enfrentarse al paciente con una profunda comprensión de su esencia social y desde una posición humanista. Objetivo: determinar las condiciones sociales del sufrimiento de los pacientes con fractura del tercio proximal del fémur durante el preoperatorio. Método: se realizó un estudio de revisión sistemática cualitativa, (...) con la información cualitativa descriptiva a través de la recolección, evaluación, verificación y síntesis de las evidencias existentes. Resultados: si está indicada la intervención quirúrgica, se asegura la prevención de la discapacidad en la población envejecida vulnerable o frágil con alto riesgo quirúrgico. La fragilidad como proceso dinámico produce la transición a mayor fragilidad y su vez conduce a un espiral de resultados adversos de salud con carácter multidimensional y progresivo. Discusión: la ética del cuidado destaca la importancia de valorar el sufrimiento del paciente, la necesidad y sensibilidad para responder de tal forma que se proteja al paciente y sus familiares de la sensación de aislamiento, abandono y discapacidad propios de la enfermedad. El cuidado es una actividad permanente y cotidiana de la vida de las personas, el cuidado por parte de los anestesiólogos incluye profesionalidad a lo cotidiano. ABSTRACT Background: the intervention of the anesthesiologists during the preoperatory of the patients with fracture of the third proximal of the femur is not reduced to the summary of scientific data of biological character; it is a current demand to face the patient with a deep understanding of its social essence and from a humanist position. Objective: to determine the social conditions of the suffering of the patients with fracture of the third proximal of the femur during the preoperatory. Method: it was carried out a study of qualitative systematic revision, with the descriptive qualitative information through the gathering, evaluation, verification and synthesis of the existent evidences. Results: if it is suitable the surgical intervention.The fragility like dynamic process produces the transition to more fragility and its time it drives to a hairspring ofadverse resultsof health with multidimensional character and progressive. Discusión: the ethics of the care highlights the importance of valuing the patient's suffering, the necessity and sensibility to respond in such a way that is protected to the patient and its relatives of the isolation sensation, abandonment of the illness. The care is a permanent and daily activity of the life of people the care on the part of the anesthesiologists. (shrink)
